Description
Required Qualifications
- Active Oregon and Washington Social Work licensure required (Washington may be in process at submission but must be active per requirements)
- BLS Certification required
- Minimum 1 year of skilled Home Health experience required
- First-time travelers accepted
- Oasis documentation experience required
- Epic experience preferred
Key Responsibilities
Assessment & Care Planning
- Conduct biopsychosocial assessments
- Perform initial patient evaluations
- Develop and revise individualized care plans
- Establish patient and caregiver goals
Case Management & Coordination
- Care coordination across interdisciplinary teams
- Discharge planning and transition support
- Participation in care conferences
- Coordination with internal and external providers
Community Resource Navigation
- Agency on Aging services
- HCBS (Home and Community-Based Services)
- Medicare/Medicaid programs
- Veterans Choice Program
- Medical transportation resources
- Medication assistance programs
Documentation & Education
- Maintain accurate clinical documentation in Epic/Oasis
- Document patient/caregiver education
- Track patient progress and responses
- Ensure compliance with coverage and regulatory standards
Patient Population
- Adults
- Geriatric patients
Work Environment & Expectations
- Home Health setting
- No weekends required
- No on-call required
- No floating between units
- Up to ~6 patient visits per day (productivity-based model)
- Travel within a ~60-mile radius (approx. 60–90 minutes daily travel)
- Must have reliable personal transportation
System & Charting
- Epic Remote Client (laptop-based)
- Rover mobile application
- Oasis documentation system (required)
