LCSW / LICSW – Clinical Social Worker (13‑Week Contract) – Hyannis, MAAssignment Details

• Duration: 13 weeks
• Schedule: 40 hours/week
• Hours: 8:00 AM – 4:00 PM
• Shifts: Rotating days, rotating weekends, occasional holidays
• Travel Pay: $2,633/week

Position Overview

The Clinical Social Worker provides essential support to patients and families within an acute care hospital setting. This role involves discharge planning, after‑care coordination, and maintaining communication with referring agencies and community resources. Candidates must be compassionate, organized, and comfortable working in a fast‑paced clinical environment.

Key Responsibilities

• Social Work Services — plan, organize, and implement social work support for patients and families
• Discharge Planning and coordination of after‑care needs
• Maintain communication with referring agencies, community partners, and care teams
• Provide emotional support, resource guidance, and crisis intervention as needed
• Document assessments, plans, and interventions in accordance with hospital standards

Requirements

• Master’s Degree in Social Work (MSW)
• Active LCSW or LICSW (Massachusetts)
• CPI (Crisis Prevention) / MAB required within 1 month of start; annual renewal required
• Ability to read, write, and communicate effectively in English
• Acute care hospital social work experience preferred
• Strong interpersonal skills, empathy, and ability to work with diverse patient populations
