Description

Pride Health is hiring an Physical Therapist Home Health to support our clients in the medical facility. This is a temporary assignment opportunity and a great way to start working with a top-tier healthcare organization!
 

Job Title -Physical Therapist Home Health

Job Type: Contract

Location- Dallas, TX

Shift:  Monday–Friday, 40 hrs/ week

Weekend Requirement: 1 Weekend per Month

Mileage Reimbursement - Yes 

 

Key Responsibilities

  • Perform comprehensive admission assessments and OASIS evaluations for home health patients.
  • Develop individualized treatment plans based on patient needs, physician orders, and clinical findings.
  • Provide skilled physical therapy interventions focused on improving strength, balance, mobility, endurance, pain management, and functional independence.
  • Treat post-operative orthopedic patients and other individuals recovering from injury, surgery, or illness.
  • Complete routine treatment visits and monitor patient progress toward established goals.
  • Educate patients, caregivers, and family members on therapeutic exercises, mobility techniques, safety precautions, and home exercise programs.
  • Accurately document patient care, evaluations, and visit notes using HomeCare HomeBase (HCHB).
  • Collaborate with physicians, nurses, occupational therapists, and interdisciplinary healthcare teams to coordinate patient care.
  • Ensure compliance with state regulations, agency policies, and home health documentation standards.
  • Participate in weekend coverage rotation as required.

     

Required Qualifications

  • Current and active Physical Therapist license in the state of Texas.
  • Minimum of 1 year of home health physical therapy experience.
  • Hands-on experience using HomeCare HomeBase (HCHB).
  • Experience treating post-operative orthopedic patients.
  • Strong knowledge of OASIS documentation and home health regulations.
  • Current CPR/BLS certification.
  • Valid driver’s license, reliable transportation, and current auto insurance.
