Description
The Registered Nurse (RN) Case Manager coordinates and oversees patient care across the healthcare continuum to ensure high-quality, cost-effective, and efficient outcomes. They assess patient needs, develop individualized care plans, and collaborate with physicians, specialists, and interdisciplinary teams. Key Responsibilities
- Conduct comprehensive patient assessments and develop personalized care plans
- Coordinate services between hospitals, home health, rehab, physicians, and payers
- Monitor patient progress, adjust care plans, and prevent complications or readmissions
- Educate patients and families on disease management, medications, and self-care
- Manage utilization of resources and ensure compliance with regulatory requirements
- Facilitate transitions of care and discharge planning
- Document all activities accurately in electronic medical records
Qualifications
- Active RN license (BSN preferred)
- 2+ years of clinical nursing experience (case management or home health experience preferred)
- Case Management Certification (CCM) a plus
- Strong communication, critical thinking, and organizational skills
