Job Overview

We are seeking a dedicated and experienced Health Home Registered Nurse to join our multidisciplinary care team. Responsibilities

• Manage case documentation within medical records systems while adhering to HIPAA regulations to maintain confidentiality.
• Provide clinical support in various settings such as hospital medicine, outpatient clinics, and post-acute care facilities.
• Collaborate with healthcare teams on diagnostic evaluations, utilization management, and treatment strategies for acute and chronic conditions.
• Monitor patient progress through ongoing assessment and modify care plans accordingly to optimize health outcomes.
• Engage in patient education regarding medication management, disease processes, and health maintenance strategies.
• Support infectious disease care initiatives and respond effectively to emergency room situations requiring immediate nursing intervention.
• Assist in managing complex cases involving behavioral health, geriatrics, pediatrics, or trauma-related needs.
• Participate in quality improvement activities focused on patient service excellence and safety standards.
