169953 – CO-Hospital Pharmacist Day/Evening Rotating Weekends – Glenwood Springs, CO 81601

 

Pay Rate: $80.96 per hour (Biweekly Pay)

Assignment Dates: 06/02/2026 – 10/20/2026

Position Type: Temporary to Permanent (opportunity to extend or go permanent based on business needs)

Location: Glenwood Springs, CO 81601, United States

Schedule: Rotating day and evening shifts, 40 hours per week
Shift Hours (Standard Time): Day and evening shifts 
Weekend Requirement: Rotating weekends (7:00 AM – 12:00 PM)
Additional Requirement: May include occasional overnight shifts as needed based on staffing needs

 

Position Overview:
The Hospital Pharmacist is responsible for preparing, dispensing, and managing medication therapies in a hospital setting while ensuring safe, effective, and cost-efficient patient care. This role requires strong clinical pharmacy expertise, Epic EMR experience, and the ability to collaborate with physicians and clinical teams in an acute care environment.

Key Responsibilities:

  • Interpret medication orders and accurately enter them into computerized patient medication profiles
  • Compound and dispense medications including IV admixtures, chemotherapy, and parenteral nutrition products
  • Ensure compliance with all federal, state, and local pharmacy regulations
  • Manage and document controlled substance distribution and inventory per legal requirements
  • Monitor medication therapy for drug interactions, contraindications, allergies, and dosing accuracy
  • Perform pharmacokinetic dosing per hospital protocols
  • Review and interpret lab data and culture & sensitivity results for antibiotic optimization
  • Assess patient medical records for clinical appropriateness of therapy
  • Identify and report adverse drug reactions
  • Support formulary management and promote cost-effective medication use
  • Provide clinical consultation and therapeutic recommendations to providers
  • Offer drug information and education to patients and families as needed
  • Accurately document all clinical interventions and pharmacy activities
  • Participate in quality improvement and medication safety initiatives
  • Support training, education, and orientation for clinical staff
  • Serve as a member of the hospital Code Response Team

Required Qualifications:

  • Active Colorado Pharmacist license in good standing required
  • Graduate of an ACPE-accredited School of Pharmacy with a BS Pharmacy degree required
  • Epic EMR experience required
  • BLS certification through the American Heart Association required
  • ACLS certification through the American Heart Association required
  • Strong clinical hospital pharmacy experience required
  • Experience with IV admixtures, chemotherapy, and sterile compounding preferred
  • Strong knowledge of medication safety, pharmacology, and clinical protocols
  • Ability to interpret lab results, medication orders, and patient charts accurately
  • Strong communication and clinical decision-making skills required
