Facility Name: Powers Health Rehabilitation Center – Crown Point, Indiana

Floor: Free-Standing Inpatient Rehab Hospital

• Role (RN, Tech, LPN, PT, etc.): Physical Therapist
• Specialty: Inpatient Rehabilitation
• Shift: Days
• Weekly Guaranteed Hours: 40
• Weekend Hours: Every Other Weekend
• Call Requirements: no
• Required Certifications: Adult Patient Population
• Years of Experience: >1-2
• Float Requirements: None

• Number of Beds: 40
• Patient Ratios: 5:1
• Equipment: Lite Gait, Biodex, Zero- G
• EHR: Epic

• Supervisor: Diana Fuentes/ Jeff Orange

• Patient Population : Adult – mix of stroke, Traumatic/Non – Traumatic Brain Injury, Debility, amputation, Non-Truamatic Spinal Cord, some traumatic ; Cardiac/Pulmonary patients; Neurological Diroders ( Myopathy, MS, Parkinsons, )

• Must Have Skills/Experience: Licensed PT in State of Indiana
• Common Diagnosis/Treatment: As above under Patient Population
• Personality Fit: cohesive team; therapy and nursing work extremely well together

• Required Education: BS/MS/DPT
