Job Title: RN Care Manager – Home Health
Location: Portland, Oregon 97216
Shift: Days | Monday–Friday | 8:00 AM – 5:00 PM
Weekly Hours: 40
Contract Length: 13 Weeks
Job Overview:
We are hiring an experienced RN Care Manager – Home Health for a 13-week travel assignment in Portland, OR. The RN will coordinate and manage patient care in home settings, collaborate with interdisciplinary teams, and ensure patients receive high-quality, goal-oriented care.
Responsibilities:
- Manage and coordinate home health patient caseloads
- Develop and implement individualized care plans
- Collaborate with physicians, therapists, and healthcare teams
- Educate patients and families on ongoing care needs
- Monitor patient progress and adjust care plans as needed
- Maintain accurate documentation and compliance standards
Requirements
- Active Oregon RN License
- Home Health or Acute Care experience preferred
- CPR or BLS certification required
- Valid Driver’s License required
- Reliable transportation and current auto insurance required
