Description
Travel Home Health RN opportunity available in Michigan for a compassionate and independent clinician who enjoys delivering one-on-one patient care in the home setting. This 13-week assignment is ideal for an experienced Home Health nurse comfortable managing autonomous field work, coordinating care across multiple counties, and building strong patient relationships in the community.

Responsibilities

• Provide skilled nursing care to patients in their homes in accordance with physician orders and plan of care
• Perform patient assessments, care coordination, and clinical documentation
• Deliver patient and family education regarding treatment plans, medications, and disease management
• Collaborate with interdisciplinary care teams including therapy services
• Manage daily visit schedules independently across assigned coverage areas
• Support safe care transitions and continuity of care

Requirements

• Active Michigan RN license required
• BLS certification required
• Minimum 1 year of Home Health RN experience required
• Strong independent assessment and time management skills
• Reliable transportation for field-based patient visits
• Previous travel experience welcomed; first-time travelers accepted

Additional Details

• 13-week contract
• 5x8 day shifts (8:00 AM–5:00 PM)
• 40 guaranteed hours weekly
• 5–6 patient visits per day
• Coverage area includes Calhoun, Kalamazoo, and Van Buren counties
• No call required
• Weekend rotation approximately every 4th weekend
• First-time travelers accepted
• Locals accepted
