Description

Shift: Day Shift – Monday–Friday (8:00 AM – 5:00 PM)
Contract Length: 13 Weeks

Required Qualifications

Active Registered Nurse (RN) license in the state of practice
CPR or BLS certification (BLS, HS‑BLS, or RQI‑BLS accepted per AH policy)
Valid driver’s license (must be 21 years or older)
Current automobile insurance

Key Responsibilities

Perform home health nursing visits and comprehensive patient assessments
Develop and manage individualized plans of care
Provide skilled nursing care including medication management, wound care, and patient education
Coordinate care with physicians, therapists, and interdisciplinary teams
Educate patients and families on disease management and treatment plans
Maintain accurate, timely documentation per agency standards
Travel within the assigned service area to patient homes
