Description

Shift: Day Shift – Monday–Friday (8:00 AM – 4:30 PM)
Contract Length: 13 Weeks

Required Qualifications

Active California RN license
Minimum 2 years acute care RN Case Manager experience
Prior travel RN experience (HARD STOP)
Epic EMR proficiency
Strong knowledge of: Discharge planning, Care coordination, DME, SNF, and transport arrangements

Key Responsibilities

Perform comprehensive patient assessments
Provide care coordination and discharge planning across acute care settings
Arrange DME, SNF placement, and ambulance transportation independently
Participate in family meetings and treatment plan development
Navigate payer systems including HMOs, IPAs, workers’ comp
Maintain regulatory compliance (CMS, DRG, HIPAA, Joint Commission)
