Description
Job Title: Registered Nurse (RN) – Home Health
Location: Urbandale, IA
Pay: $54–$57 per hour | Paid Bi-Weekly
Responsibilities
- Administer skilled nursing care to patients of all ages in their homes
- Assess patients’ physical, psychosocial, and environmental needs
- Develop, implement, document, and evaluate individualized Plans of Care (POC)
- Educate patients and families on medications, diet, treatment plans, and home safety
- Supervise and provide clinical guidance to HHA/STNA/LVN staff
- Maintain accurate and timely patient documentation
- Ensure compliance with healthcare regulations and third-party payer requirements
- Participate in on-call rotation as needed based on branch and staffing needs
- Coordinate care with interdisciplinary healthcare teams
Qualifications
- Current Registered Nurse (RN) license in the state of Iowa
- Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program
- Current CPR certification required
- CHPN certification preferred
- Previous home health or hospice experience preferred
- Strong assessment, communication, and organizational skills
- Ability to work independently and manage a flexible schedule
