Description
RN Home Health Case Manager
Location: Simi Valley, CA 93062
Position Details:
- Shift: Day Shift, Monday–Friday, 8:00 AM – 5:00 PM
- Rotating weekends required as part of the 40-hour schedule
- EPIC experience required
- Minimum 2 years of recent Home Health/related specialty experience within the last 3 months required
Requirements:
- Active RN license in the state of practice
- BLS certification
- Valid Driver’s License
- Must be 21 years or older
- Current automobile insurance and reliable vehicle required
