Registered Nurse – Home Health / Hospice

Pay: $47–$51/hour, paid weekly

Location: Local travel required within assigned service area

Hours: Flexible scheduling based on patient needs with on-call rotation responsibilities as assigned

Key Responsibilities:

• Assess home health and/or hospice patients in stable and emergent situations
• Develop, implement, document, and evaluate individualized Plans of Care (POC)
• Coordinate patient care with physicians, caregivers, and interdisciplinary team members
• Maintain accurate and timely clinical documentation and patient records
• Provide patient and family education regarding medications, treatments, home safety, diet, and self-care
• Utilize infection control practices in accordance with OSHA standards
• Assess and manage patient pain while educating patients and families on pain management techniques
• Supervise and provide clinical direction to support staff including HHA/STNA/LVN team members
• Perform clinical nursing duties including:
•  
  • Wound care and treatment
  • IV care and venipuncture
  • Inhalation therapy
  • Phlebotomy
  • Catheter and drain care
  • Bowel and bladder training programs
• Coordinate referrals to appropriate healthcare agencies and services as needed
• Participate in interdisciplinary meetings and collaborate with healthcare professionals to ensure quality patient care
• Monitor assigned cases for compliance with payer and regulatory requirements
• Perform additional duties as assigned

Qualifications:

• Current state Registered Nurse (RN) license required
• Current CPR certification required
• Valid driver’s license required
• Reliable transportation and current auto insurance required
• 1–2 years of Registered Nurse experience in a clinical care setting required
• 2+ years of home health experience required
• Homecare Homebase software experience required
• OASIS experience required
• CHPN certification preferred
• Strong clinical, communication, and organizational skills
• Ability to work independently and collaboratively within a team environment
• Ability to travel locally and maintain a flexible schedule

Education

• Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program required
• Bachelor of Science in Nursing (BSN) preferred
