Description

Position: RN Care Manager (Home Health)

Facility: Adventist Health Simi Valley

Location: 2975 Sycamore Dr, Simi Valley, CA 93065

Contract Duration: 13 weeks (with possible extension)

Schedule: (5x8) Day shift (8:00 am -4:30 pm)

Weekly Hours: 40 hours

Job Description

The Home Health Registered Nurse is responsible for planning, coordinating, and delivering high-quality home care services tailored to meet patients’ individual healthcare needs in their home environment. The RN will effectively manage a patient caseload while providing independent nursing care and collaborating with interdisciplinary healthcare teams to ensure patient care goals are achieved. This role also serves as a key resource for patients, family members, and care team members by offering guidance, education, and support throughout the care process.

Requirements

Day Shift: Monday–Friday, 8:00 AM – 5:00 PM
Rotating weekends required as part of the 40-hour work schedule
EPIC EMR experience required

Education & Experience

Bachelor of Science in Nursing (BSN) preferred
Prior acute care or home health experience preferred
