Job Overview

We are seeking an experienced Home Health Registered Nurse (RN) for a long-term contract opportunity in Portage, Indiana. This role focuses on delivering high-quality patient-centered care in the home health setting while collaborating closely with interdisciplinary healthcare teams and caregivers.

The ideal candidate will have strong clinical assessment skills, experience with patient education and care coordination, and the ability to work independently in a home health environment.

Key Responsibilities

• Perform comprehensive patient assessments and ongoing evaluations
• Develop, implement, and update individualized care plans
• Educate patients and caregivers on chronic disease management, medications, and self-care techniques
• Coordinate care with physicians, therapists, social workers, and interdisciplinary teams
• Monitor patient progress and communicate condition changes appropriately
• Maintain accurate EMR documentation including assessments and progress notes
• Supervise caregivers, home care aides, and support staff to ensure quality patient care
• Ensure compliance with agency policies, care plans, and healthcare regulations

Requirements

• Active RN License (Indiana Compact License Preferred)
• Previous Home Health or Case Management experience preferred
• Strong communication, organizational, and patient education skills
• EMR documentation experience required
• Ability to work independently and collaboratively within a healthcare team
• Associate Degree in Nursing required
• BSN preferred
