LPN - Continuing Care
Location: Albemarle, NC 28001
Setting: Skilled Nursing Facility
Shift: 12-hour Night shifts
Travel Requirement: Candidate’s permanent address must be at least 50 miles from the home unit.

Seeking an experienced LPN for a Continuing Care assignment in a 90-bed skilled nursing facility serving short-term rehab and long-term care patients. This unit cares for patients recovering from illness, injury, or accident, including stroke, amputation, neurological disorders, and post-joint replacement cases.

Facility details

• 90-bed Continuing Care unit in a Skilled Nursing Facility.
• First-time travelers are not preferred.
• Minimum 18 months of SNF/LTC rehab experience required.
• No memory care patients on this unit.

Requirements

• Active LPN license and BLS required.
• Recent long-term care experience required, including trach care, foley care, wound care, dressing changes, IV starts/management, and tube feedings.
• Trach refresher is provided during orientation.

Unit support

• Nursing leadership, admin nurse, HCT, therapies, and case management support available.
• Admin nurse handles admissions, with approximately 1 to 2 admissions per day.
• EMR: Epic; IV Pump: Alaris; Med Dispensing: Omnicell.

Scheduling and floating

• Ratios: LPN 1:18-20 on days and 1:26 on nights.
• Floating is required within Continuing Care and across the rehab region based on staffing needs.
• Regional facilities include Cleveland Pines, Huntersville Oaks, Jesse Helms, Sardis Oaks, and Stanly Manor; travelers remain on the clock for mid-shift travel between sites.

• Orientation includes 2 departmental days.
• Scrub color: Ceil Blue.
• Free parking available.
