Advanced Practice Pharmacist I / II – Job Description
Position Overview
The Advanced Practice Pharmacist I / II is responsible for delivering comprehensive pharmacy and medication management services for inpatient and outpatient populations at Tuba City Regional Health Care Corporation (TCRHCC). The role supports direct patient care, clinical pharmacy services, medication safety, and pharmacy-managed specialty clinics.
This position works collaboratively with physicians, nurses, and healthcare staff to ensure safe, effective, and evidence-based medication therapy for neonatal, pediatric, adolescent, adult, geriatric, oncology, and obstetric patients.
The position may require rotational shifts including days, evenings, nights, weekends, and holidays.

Key Responsibilities
Clinical Responsibilities
- Review and verify medication orders for accuracy, appropriateness, dosage, route, and interactions.
- Provide direct patient care and medication management services.
- Monitor patient outcomes and adjust treatment plans under approved pharmacy clinic protocols.
- Review chemotherapy treatment plans and ensure safe oncology medication administration.
- Counsel patients and families regarding medication use, side effects, compliance, and treatment plans.
- Collaborate with physicians and nursing staff regarding pharmacotherapy recommendations.
- Monitor for drug-drug, drug-allergy, and drug-disease interactions.
- Participate in pharmacy-managed specialty clinics.
- Order and interpret laboratory tests as appropriate for clinical pharmacy services.
- Supervise pharmacy technicians and support staff.
- Maintain compliance with USP 800 regulations and sterile/non-sterile compounding standards.
- Participate in emergency preparedness, pandemic response, and disaster drills.
- Maintain accurate controlled substance records and pharmacy documentation.
Leadership & Operational Responsibilities (Advanced Practice Pharmacist II)
- Manage and oversee pharmacy-based specialty clinics.
- Develop and implement clinical pharmacy programs and quality initiatives.
- Assist in policy development and operational planning.
- Monitor departmental clinical performance indicators and compliance standards.
- Provide training and mentorship to pharmacists, technicians, interns, and healthcare professionals.
- Participate in performance improvement and quality assurance activities.
- Support cost management initiatives within pharmacy services.
Required Qualifications
Education
- Doctor of Pharmacy (PharmD) or Bachelor of Pharmacy degree from an ACPE-accredited institution.
Licensure & Certifications
- Current and unrestricted Pharmacist license in any U.S. State or Territory.
- Current Basic Life Support (BLS) certification through the American Heart Association.
Experience:
Advanced Practice Pharmacist I
- Minimum 2 years of experience as a Registered Pharmacist.
- Minimum 1 year of Clinical Pharmacist experience.
Advanced Practice Pharmacist II
- Minimum 5 years of experience as a Registered Pharmacist.
- Minimum 2 years of Clinical Pharmacist experience.
Preferred Qualifications
- PGY-1 Residency preferred.
- Ability to speak Navajo, Hopi, or San Juan Southern Paiute preferred.
Required Skills & Competencies
- Strong clinical pharmacy knowledge and pharmacotherapy expertise.
- Knowledge of pharmacokinetics, pathophysiology, and treatment protocols.
- Experience with sterile and hazardous medication compounding.
- Understanding of federal pharmacy regulations and medication safety standards.
- Excellent communication and patient counseling skills.
- Ability to work independently in high-pressure clinical environments.
- Strong organizational, leadership, and problem-solving abilities.
- Proficiency with pharmacy software systems and electronic medical records.
Physical & Work Environment Requirements
- Frequent standing, walking, and prolonged computer use.
- Ability to lift up to 10 pounds.
- Exposure to hazardous medications, chemicals, and infectious materials.
- Ability to work flexible schedules, including weekends and holidays.
- Ability to perform duties in high-stress healthcare environments.
