Description
- Schedule: 8:00 AM – 5:00 PM
- Shift: Days, Monday–Friday (5 shifts/week)
- Start Date: ASAP
Requirements:
- Minimum 1 year of experience in a Medicare‑certified home health agency
- Proficiency with OASIS documentation
- Experience with HCHB EMR
- Active RN license and willingness to obtain Florida licensure
Key Responsibilities
- Comprehensive Assessments — Evaluate patient health status and develop individualized care plans
- Skilled Nursing Care — Provide wound care, medication management, infusion therapy, and other clinical interventions
- OASIS Documentation — Complete accurate, timely assessments to meet regulatory standards
- Patient & Family Education — Teach disease management, medication administration, and self‑care strategies
- Care Plan Management — Monitor progress and adjust interventions to support optimal outcomes
- Emotional Support — Assist patients and families coping with chronic illness or end‑of‑life needs
This mobile, autonomous role keeps you connected to the broader care team — including physicians, therapists, and social workers — while giving you the independence many clinicians love.
