Licensed Master Social Worker (LMSW) – Case Manager
Location: Echo Cliffs Healthcare Center / Tuba City Regional Health Care Corporation

Reports To: Director of Care Coordination
POSITION SUMMARY
The Licensed Master Social Worker (LMSW) – Case Manager is responsible for coordinating comprehensive outpatient case management services for high-risk patients at Echo Cliffs Healthcare Center. This role focuses on assessment, care planning, resource coordination, discharge planning, and continuity of care to improve patient outcomes and quality of life.
The LMSW Case Manager collaborates with healthcare providers, social service agencies, behavioral health programs, and community organizations to ensure patients receive appropriate medical, social, and supportive services. The position promotes patient self-sufficiency, reduces fragmentation of care, and supports cost-effective utilization of healthcare resources.
KEY RESPONSIBILITIES
- Conduct comprehensive psychosocial assessments for high-risk patients
- Develop individualized patient-centered care and service plans
- Coordinate referrals to community resources, behavioral health, substance abuse programs, and social service agencies
- Collaborate with physicians, nurses, PRC staff, utilization review teams, and external healthcare providers
- Facilitate discharge planning and continuity of care services
- Monitor patient progress and update care plans as needed
- Assist patients and families with accessing healthcare and community support services
- Provide advocacy, education, and supportive counseling to patients and families
- Participate in multidisciplinary care coordination meetings
- Maintain accurate documentation within electronic health record systems
- Support utilization review and resource management activities
- Promote patient self-advocacy and informed decision-making
- Ensure compliance with organizational policies, CMS guidelines, and patient confidentiality standards
- Participate in weekend on-call coverage as assigned
- Perform additional duties as assigned
REQUIRED QUALIFICATIONS
Education
- Master’s Degree in Social Work (MSW) from an accredited institution
License & Certification
- Current Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) license in any U.S. state or territory
- Current Basic Life Support (BLS) Certification through the American Heart Association
Experience
- Minimum three (3) years of clinical or healthcare-related experience
- Experience in case management, discharge planning, or utilization review preferred
- Experience working with diverse patient populations and community resources
- Knowledge of electronic health record (EHR) systems
PREFERRED QUALIFICATIONS
- Hospital or clinic case management experience
- Experience with Medicare, Medicaid, AHCCCS, or PRC processes
- Knowledge of Patient-Centered Medical Home (PCMH) models
- Experience working with underserved or culturally diverse populations
- Bilingual skills in Navajo, Hopi, or San Juan Southern Paiute preferred

PHYSICAL REQUIREMENTS
- Ability to sit, stand, walk, bend, and lift up to 50 pounds occasionally

WORK ENVIRONMENT
May involve exposure to infectious diseases, hazardous materials, and variable environmental conditions. Proper use of Personal Protective Equipment (PPE) is required.
