Job Title: LPN – Inpatient Rehab (Day Shift, Block Schedule Fri–Sun)

Job Description:

We are seeking a dedicated LPN for a full-time day shift position at a 66-bed inpatient acute rehabilitation hospital in Fort Worth, TX. This facility specializes in the treatment and recovery of patients who have experienced serious injuries or illnesses requiring intensive rehabilitation.

This is a block schedule role (Friday, Saturday, Sunday) with 12-hour day shifts (06:45–19:15). The LPN will provide direct patient care in a rehabilitation setting focused on restoring function and improving quality of life.

Patient population includes:

• Stroke recovery
• Brain injury
• Spinal cord injury
• Neurological conditions
• Trauma and orthopedic recovery
• Amputations

Responsibilities include:

• Providing skilled nursing care in an inpatient rehab setting
• Monitoring patient progress and documenting care in EMR (Cerner)
• Supporting therapy-driven recovery plans
• Collaborating with RN staff, therapists, and providers
• Administering medications and following care plans

Requirements:

• Active TX LPN/LVN license (or compact)
• BLS certification required
• Minimum 1 year nursing experience required
• Experience in rehab, SNF, or acute care preferred
