Description

Role Responsibilities:

Evaluate, treat, and rehabilitate patients with physical function disorders and wound care needs while developing individualized care plans.
Perform rehabilitation procedures including therapeutic exercises, manual therapy, ambulation training, modalities, assistive device support, and wound care.
Monitor patient progress, modify treatment plans, and document all clinical activities in compliance with regulatory and company standards.
Provide patient and caregiver education, discharge planning, and coordinate with healthcare teams regarding treatment progress and outcomes.
Participate in meetings, trainings, and supervise physical therapist assistants, aides, and students while adhering to APTA and state practice guidelines.

Requirements

1 year of PT experience
New Grads OKAY- as long as a Level II experience and not just a Level I observation experience
LTC or Rehab experience
