Description

Role Responsibilities:

Evaluate, treat, and rehabilitate patients with communication, cognitive, and swallowing disorders while developing individualized care plans.
Conduct standardized assessments and apply evidence-based interventions to support patient progress.
Monitor, document, and adjust treatment plans based on patient outcomes and regulatory requirements.
Provide counseling and education to patients, families, caregivers, and healthcare teams regarding speech and swallowing disorders.
Maintain accurate electronic documentation, treatment records, and discharge summaries in compliance with ASHA, state, and company standards.
Participate in patient care meetings, trainings, and interdisciplinary collaboration to ensure quality rehabilitation services.
Supervise and support CFY and student Speech-Language Pathologists while maintaining active ASHA certification and state licensure.

Requirements

1 year of SLP experience
LTC or Rehab experience
MA State License
CCC certified SLP
New Grads OKAY- as long as a Level II experience and not just a Level I observation experience
