Description
Role Responsibilities:
- Evaluate, treat, and rehabilitate patients with communication, cognitive, and swallowing disorders while developing individualized care plans.
- Conduct standardized assessments and apply evidence-based interventions to support patient progress.
- Monitor, document, and adjust treatment plans based on patient outcomes and regulatory requirements.
- Provide counseling and education to patients, families, caregivers, and healthcare teams regarding speech and swallowing disorders.
- Maintain accurate electronic documentation, treatment records, and discharge summaries in compliance with ASHA, state, and company standards.
- Participate in patient care meetings, trainings, and interdisciplinary collaboration to ensure quality rehabilitation services.
- Supervise and support CFY and student Speech-Language Pathologists while maintaining active ASHA certification and state licensure.
Requirements
- 1 year of SLP experience
- LTC or Rehab experience
- MA State License
- CCC certified SLP
- New Grads OKAY- as long as a Level II experience and not just a Level I observation experience
