Position Overview:
We are seeking an experienced and dedicated Home Health Registered Nurse to provide high-quality, patient-centered care to individuals throughout Baltimore County and Baltimore City. This role focuses on delivering skilled nursing services in a home setting, managing complex clinical needs, and supporting patients in achieving optimal health outcomes.

Schedule & Work Details:

• Location: Baltimore County and Baltimore City
• Schedule: Day shift (specific hours to be confirmed)
• Weekly Guarantee: 40 hours
• On-Call/Weekend Requirement: 2 days
• Mileage Reimbursement: Yes, through contract company (between patient visits from first to last patient)
• Pay Rate: $51 to $53 per hour

Key Responsibilities:

• Provide skilled nursing care to patients in their homes in accordance with physician orders and plan of care
• Complete comprehensive assessments including OASIS documentation
• Administer IV therapies, manage PICC lines, and perform wound care treatments
• Monitor patient conditions and identify changes, escalating concerns appropriately
• Manage chronic diseases and medication regimens
• Educate patients and caregivers on disease processes, medications, and home care techniques
• Coordinate care with physicians and interdisciplinary team members
• Maintain accurate, timely, and compliant documentation

Required Qualifications:

• Active Registered Nurse (RN) license
• Prior home health experience required
• Experience with OASIS documentation required
• Competency in IV therapy, PICC line care, and wound care
• Strong clinical assessment and documentation skills
• Ability to work independently in a field-based environment
