Job Summary:
The Licensed Practical Nurse (LPN) provides basic nursing care to patients under the supervision of a Registered Nurse (RN) or healthcare provider. The LPN supports patient care by collecting health information, assisting with assessments, and following established care plans. This role involves working closely with the healthcare team to ensure patients receive safe and effective care.
Key Responsibilities:
- Provide direct patient care as assigned
- Monitor and record patient conditions and vital signs
- Assist in patient assessments and communicate findings to the RN/provider
- Follow care plans, policies, and procedures
- Support patients and families with care needs and education
- Maintain accurate documentation in medical records
- Collaborate with the healthcare team to deliver quality care
