Shift: Day Shift ( 3 Days in a week)
Certification: BLS (AHA)
Perform comprehensive assessments of patients’ needs and coordinate individualized care plans.
· Provide pain and symptom management consistent with the hospice plan of care.
· Educate and support patients and families regarding the disease process, self-care techniques, end-of-life issues, and the hospice philosophy.
· Collaborate with the interdisciplinary team including physicians, social workers, chaplains and volunteers.
· Participate in on-call rotation
· Maintain accurate and timely documentation in compliance with state, federal, and organizational policies
