Summary

We are seeking an experienced Home Health RN for a skilled home care travel assignment. The ideal candidate will have strong OASIS experience, skilled nursing home visits experience, and the ability to independently manage patient care in home settings.

Responsibilities

• Perform skilled nursing visits in patient homes (6–8 visits per day)
• Conduct cardiopulmonary assessments and patient education
• Manage wound care including:
  • Basic wound care
  • ABI compression dressings
  • Wound vac dressing changes
• Provide IV therapy and PICC/Midline care
• Perform:
  • Foley catheter changes and care
  • SQ/IM injections
  • Venipuncture and lab draws
  • Port access/de-access
  • Staple and suture removal
• Monitor and manage high-risk diagnoses including:
  • Heart Failure (HF)
  • COPD
  • MI
  • Pneumonia
  • Stroke
• Support post-operative patients and chronic disease management
• Complete accurate documentation using BayCare systems and OASIS

Required Qualifications

• Minimum 1 year of Skilled Home Health experience
• OASIS experience required
• BLS certification required
• Strong IV and wound care skills
• Valid driver’s license and reliable vehicle required
• COVID vaccination or approved exemption required
• Flu shot or signed declination required
• Must wear a mask while working

Skills & Experience

• Blood glucose/glucometer management
• Phlebotomy and IV management
• Starting and maintaining IVs
• Medication administration:
  • Intramuscular injections
  • Subcutaneous injections
• Skilled wound care experience

Equipment & Documentation

• Laptop, MiFi, and portable printer provided
• Documentation systems include:
  • BayCare Forms
  • BayCare Scheduling
  • Ace Timesheet
