Schedule: 0730–1600 or 0800–1630
Guaranteed Hours: 40 Hours/Week

Requirements:

Active RN License
Minimum 2 years of recent Palliative Care and/or Hospice experience
Acute Care experience required
BLS Certification required
Strong clinical judgment, communication, and organizational skills

Job Responsibilities:

Manage inpatient palliative care caseload
Conduct patient assessments and chart reviews
Lead goals-of-care and end-of-life discussions
Educate patients and families on disease processes, POLSTs, and advance directives
Participate in interdisciplinary rounds and family meetings
Coordinate care plans with physicians and care teams
