Job description:

Position: LPN
Unit : LTC
Shift: 3 x 12 hrs,Evenings (2 PM - 10 PM)
Duration: 13 weeks
Location: New Lisbon, WI 53950

Travel Pay Rate: (above 50 miles)
Hourly: $20/hr W2 + $910/week (non - taxable stipend)
Gross: $1,630/weekly

Local Pay Rate: (within 50 miles)
Hourly: $40/hr (W2)

Job Summary:
Licensed Practical Nurse (LPN) – Long-Term Care (LTC) provides direct nursing care to residents in long-term care facilities, ensuring comfort, safety, and quality of life. Responsible for medication administration, monitoring resident conditions, assisting with daily care needs, and supporting individualized care plans under RN or physician supervision.

Roles & Responsibilities:

• Assess and monitor residents’ health status and vital signs
• Administer medications and treatments as prescribed
• Provide wound care and basic nursing procedures
• Assist residents with activities of daily living (ADLs)
• Document patient care accurately in EMR/EHR systems
• Report changes in resident condition to RN, physician, and care team
• Ensure infection control and patient safety standards are followed
• Educate residents and families on care plans and treatments
• Respond to emergencies and provide immediate nursing interventions
• Collaborate with interdisciplinary teams to support resident-centered care planning

Required Qualifications:

• Active LPN license of WI or Multistate.
• BLS required.
• Minimum 1 year of experience
• Meditech experience
