Description
- 2 years inpatient pharmacist experience - [REQUIRED]
- Able to work in the Anticoagulation Clinic (4-6 pts a day) and perform Oncology compounding (pharmacist do all the IV compounding) - [REQUIRED].

- Medication Order Verification: Review and interpret electronic physician orders to confirm accurate drug choices, safe dosages, and proper administration routes.
- Clinical Monitoring & Interventions: Evaluate patient profiles for allergies, drug-drug interactions, and lab results. They frequently perform pharmacokinetic dosing for high-risk medications.
- Compounding & Dispensing: Oversee or personally prepare specialized medications, including large-volume parenterals, intravenous (IV) piggybacks, total parenteral nutrition (TPN), and chemotherapy infusions under strict sterile guidelines (such as USP <797>/<800>).
- Interdisciplinary Collaboration: Participate in medical rounds with physicians and nurses to recommend evidence-based therapies and assist in medical emergencies.
- Transitions of Care: Conduct medication reconciliation during a patient's admission and provide comprehensive drug counseling to patients upon hospital discharge.
- Staff Supervision: Manage, review, and delegate tasks to pharmacy technicians, interns, and students to maintain accurate inventory and cart distributions
