Position Summary
The Community Living Center (CLC) Registered Nurse (RN) provides skilled nursing care and rehabilitation support to Veterans in a short-stay post-acute inpatient setting at the Louis Stokes Cleveland VA Medical Center. The CLC environment is designed to promote rehabilitation, recovery, and safe transition back to the home or community setting following hospitalization, surgery, or acute illness.
The RN delivers comprehensive nursing care to medically stable patients requiring skilled interventions, rehabilitation support, chronic condition monitoring, and interdisciplinary discharge planning. The nurse collaborates with physicians, therapists, case managers, and support staff to ensure high-quality Veteran-centered care.
CLC Unit Areas May Include
Coral Cove (CT2)
37-bed short-stay rehabilitation neighborhood focused on helping Veterans regain prior functional status following acute medical hospitalization.
Admiral’s Point (3B)
14-bed post-acute respiratory-focused neighborhood caring for Veterans requiring respiratory support beyond what can safely be managed at home.
Waterside Way (CT3) & Emerald Woods (CT4)
Short-stay rehab and skilled nursing neighborhoods providing rehab care, IV antibiotic management, and simple to complex wound care.
Soaring Heights (CT5)
Secure dementia/cognitive impairment neighborhood providing holistic care for Veterans requiring respite, rehab, placement assistance, or medical support.
Blind Rehab (WCTA)
Certified rehabilitation center assisting visually impaired Veterans in achieving independent living goals.
Heroes Harbor Hospice
12-bed hospice neighborhood focused on symptom management, comfort care, quality of life, and interdisciplinary end-of-life support for Veterans and families.
Essential Duties & Responsibilities
- Perform comprehensive patient assessments and ongoing monitoring
- Administer medications, IV therapy, and central/peripheral line medications
- Draw laboratory specimens and monitor diagnostic results
- Provide wound care, tracheostomy care, tube feedings, and Foley catheter care
- Assist patients with activities of daily living (ADLs)
- Coordinate care with interdisciplinary teams including physical, occupational, and speech therapy
- Complete admissions, discharges, and internal/external patient transfers
- Delegate tasks appropriately to assistive personnel
- Document patient care accurately in the electronic medical record (EHR)
- Participate in emergency response procedures and patient transport as needed
- Identify barriers to rehabilitation and collaborate with the care team to support safe discharge planning
- Participate in quality improvement initiatives and reporting as requested
- Float between CLC units based on operational and staffing needs
Qualifications
- Current, unrestricted Registered Nurse (RN) license
- BLS certification required
- ACLS may be required based on assignment
- Minimum clinical experience in skilled nursing, rehab, long-term care, med-surg, or post-acute care preferred
- Ability to work independently and within an interdisciplinary team
- Strong organizational, communication, and critical-thinking skills
- Experience with electronic medical records preferred
