Description
Travel Home Health Physical Therapist – Greenbelt, MD
Location: Greenbelt, MD
Contract Length: 13 Weeks
Shift: Days | 5x8s | 8:00 AM – 5:00 PM
Mileage Reimbursement Available
Requirements:
Minimum 2 years of Home Health PT experience required.
Recent Homecare Homebase (HCHB) and OASIS experience required.
Active MD PT License and BLS Certification required.
First-time travelers accepted with strong OASIS experience.
Candidates must have a reliable personal vehicle with valid insurance and registration.
Schedule & Coverage:
Day Shift – 5x8s (08:00 AM – 05:00 PM). No call or weekend requirements.
Float within assigned coverage areas may be required based on patient needs.
Coverage Areas:Montgomery County, Prince George’s County, Charles County, and St. Mary’s County.
Responsibilities:
Provide skilled physical therapy services in patients’ homes.
Complete OASIS assessments and documentation accurately.
Develop and implement individualized treatment plans.
Educate patients and families on home exercise programs and safety techniques. Assess mobility, pain, skin integrity, and functional status.
Collaborate with interdisciplinary healthcare teams.
