Description

Position: Pharmacist – Acute Setting


Schedule: 32 hrs/week; 8-hour day shifts (likely Mon–Fri, approx. 8a–5p; schedule TBD)

Setting: 31-bed critical illness recovery hospital (post-acute rehab)

This is a 13-week contract (almost always extends) with a start date ASAP (2–3 weeks for credentialing). On-call is minimal and compensated, with very limited weekend/holiday coverage as needed.

Key Responsibilities:

  • Maintain pharmacy drug supply and manage procurement, including emergency meds
  • Medication reconciliation (admission & discharge)
  • Discharge counseling with patients/families
  • Interpret culture results and provide clinical recommendations
  • Manage pharmacokinetics and anticoagulation dosing/monitoring
  • Participate in QA programs and interdisciplinary meetings
  • Strong clinical knowledge (UTI, pneumonia, STI)

Requirements:

  • Active SC Pharmacist license
  • BLS (AHA preferred; willing to obtain if needed)
  • 2+ years recent acute care hospital experience (LTAC/rehab may be considered)
  • EPIC experience highly preferred
