Location: Anchorage, AK 99508
Job Type: 13-week contract + extensions
Shift Time:Day (07:30 - 16:00)
Working Hours: 40(5x8) hrs/week;

Position Summary:
We are seeking a dedicated and experienced RN Case Manager is responsible for coordinating and managing patient care to ensure quality treatment, cost-effective services, and positive patient outcomes. The RN Case Manager works closely with physicians, nurses, patients, families, and insurance providers to develop and implement individualized care plans throughout the patient’s treatment and recovery process.
Key Responsibilities:
- Assess patient needs and develop individualized care plans
- Coordinate patient care with healthcare teams and providers
- Monitor patient progress and treatment outcomes
- Manage discharge planning and transition of care
- Ensure compliance with healthcare regulations and insurance requirements
- Communicate with patients and families regarding treatment plans and resources
- Perform utilization review and case documentation
- Advocate for patient needs and quality care services
- Maintain accurate and timely medical records and reports
- Collaborate with physicians, social workers, and rehabilitation teams
Required Qualifications:
- AK RN license
