Description

Pride Health is hiring a Medical Assistant to support our client's patients in Centralia, WA (Coverage: 50-mile radius). This is an 11-week assignment (Possibility of extension or conversion) and a great way to start working with a top-tier healthcare organization!
 
Job Details:
Job Name: Home Health Occupational Therapist
Contract type: Local Contract
Location: Centralia, WA (50-mile radius)
Duration: 11+ week Contract (Possibilities of extension)
Pay rate: $48-53/hr. On W2 (Pay Rate is based on experience)
Schedule: Mon – Fri (5 days/wk.) 8 am to 4:30 pm (40 hrs. guaranteed)
Mileage: Reimbursement provided (from first patient to last patient)
 
Job Responsibilities:

  1. Conduct comprehensive occupational therapy evaluations and patient care assessments in the home health setting.
  2. Develop individualized treatment plans based on patient needs, physician orders, and functional goals.
  3. Complete recertifications and reassessments to evaluate patient progress and determine continued eligibility for services.
  4. Perform discharge evaluations and provide recommendations for ongoing care, home safety, adaptive equipment, and caregiver education.
  5. Deliver therapeutic interventions focused on improving mobility, strength, ADLs, safety awareness, and independence within the home environment.
  6. Maintain timely and accurate clinical documentation in accordance with agency, state, and federal guidelines.
  7. Collaborate with physicians, nurses, case managers, and interdisciplinary care teams to coordinate patient care.
  8. Educate patients and caregivers on therapy exercises, home modifications, fall prevention, and use of adaptive equipment.
  9. Ensure compliance with Medicare home health regulations and agency standards.

 
Qualifications:

  • Current Occupational Therapy license in the state of practice.
  • Home Health experience required.
  • Experience with Homecare Homebase required.
  • Current CPR certification.
  • Valid driver’s license, auto insurance, and reliable transportation.
