Description

Position Overview:
Seeking a dedicated Registered Nurse to provide skilled nursing care in a home health setting. This role focuses on delivering high-quality patient care through OASIS assessments and routine home visits across multiple counties, supporting patient independence and positive health outcomes.

Schedule & Work Details:

  • Location: Field-based role serving Warren, Edmonson, Hart, and Butler Counties
  • Schedule: Monday through Friday, 8:00 AM to 4:30 PM
  • On-Call Requirements: On call 2 to 3 nights per week and one weekend every 3 to 4 weeks
  • Weekly Hour Guarantee: 40 hours
  • Mileage Reimbursement: Yes, reimbursed between patient visits (first to last patient)
  • Pay Rate: $51 to $53 per hour

Key Responsibilities:

  • Conduct OASIS assessments in accordance with home health regulations and agency standards
  • Perform routine skilled nursing visits, including patient assessments, care planning, and interventions
  • Monitor and document patient progress, updating care plans as needed
  • Provide patient and family education to promote health management and independence
  • Collaborate with interdisciplinary team members to coordinate care
  • Maintain accurate, timely, and compliant clinical documentation
  • Participate in on-call rotation and provide after-hours support as required

Required Qualifications:

  • Active Registered Nurse (RN) license in the state of Kentucky
  • Ability to practice independently in a home health setting
  • Strong clinical assessment and documentation skills
  • Reliable transportation and ability to travel within assigned coverage areas

Preferred Qualifications:

  • Prior home health experience
  • OASIS documentation experience
  • Strong time management and organizational skills
  • Excellent communication and patient education abilities
