Schedule: 08:00 am to 04:30 pm, Monday-Friday
Location: Wilmington, MA, USA, 01887
Job Type: Travel contract
Contract Length: 15 weeks
Pay Range: Pay range: $40.00 to $43.00 an Hour
*Pay offered is based on experience, expertise, credentialing, and education.
Duties:
- Provide home health physical therapy treatments and therapeutic exercises under the supervision of a licensed Physical Therapist, following the established plan of care to improve patient mobility, strength, balance, and functional independence.
- Monitor and document patient progress, response to treatment, and any changes in condition, communicating updates promptly to the supervising Physical Therapist and interdisciplinary care team.
- Educate patients and caregivers on prescribed exercise programs, safety techniques, use of assistive devices, fall prevention, and strategies to support recovery and independence in the home environment.
- Maintain accurate and timely clinical documentation, adhere to agency policies and state regulations, participate in care coordination activities, and support quality patient care across the assigned service area.
Requirements:
- MA Physical Therapist Assistant License.
- Home Care Experience with HCHB experience.
- Type of certification required: BLS
- Minimum Education Requirement: Associate degree.

- Coverage Areas: Gloucester, Manchester By-The-Sea, Beverly, Danvers, Peabody, Salem, Marblehead, Lynnfield, Lynn, Swampscott.

- On Call/Weekend Requirement: No
- Weekly Hour Guarantee: 30 hours/week
