Hospice Liaison (RN) – CH WI Hosp Divisional
Job Summary
Location/Territory: Northern Area (Green Bay, Oshkosh, Manitowoc, Sheboygan, Grafton).

Note: Assigned to 1–2 hospitals depending on size and census. Will not cover the full territory in a single day (e.g., Green Bay/Oshkosh one day, Manitowoc/Sheboygan another).

Potential Sites: Aurora Sinai, Aurora St. Luke's, Aurora West Allis, Aurora Grafton, Aurora Hartford, Aurora St. Luke's South Shore, and Aurora Summit Hospitals.

Daily Census: 2–3 patients per day.

Candidate Requirements & Preferences
Experience: Minimum 2 years of experience required. Hospice liaison experience is highly preferred, but open to other strong hospice experience.

Travelers: First-time travelers are accepted.

EMR Requirement: EPIC experience is strictly required (specifically using EPIC Hyperspace and EPIC Remote Client for charting and secure chat).

Transportation: Must have a valid Driver's License, active Auto Insurance, and a reliable personal vehicle.

Note: Laptop and cell phone will be provided by the facility.

Clinical Role & Skills Required
Role Responsibilities
Monitor the queue and consult with patients referred from hospitals or Skilled Nursing Facilities (SNFs) for hospice care.

Provide assessments and recommendations on the proper hospice setting for patients transitioning to hospice within a hospital environment.

Admit patients directly to hospice services in the hospital setting.

Patient Types & Diagnoses
Adult populations.

Heart failure, cancer, and other end-of-life disease processes.

Core Skills Required
Symptom management for end-of-life care (pain, shortness of breath).

High comfort level with end-of-life medications and educating healthcare providers on managing them.

Strong collaboration and communication skills with physicians.

Ability to confidently provide education on hospice benefits and philosophy.

Deep understanding of the different levels of hospice care (general inpatient, routine, respite) and the various environments where hospice services can be provided.

Unit Support & Resources
Leadership: Supervisor/Manager available via phone and chat.

Peer Support: Full Liaison team available for questions.

Medical Support: Hospice Medical Directors.

Logistics & Orientation
Orientation: Full Nursing Engagement Orientation (NEO) plus 40 hours of dedicated unit orientation alongside a staff member.

Dress Code: Business casual or any professional scrubs. A white coat is preferred but not required.

Parking: Free parking available at sites.
