Job Title: LPN / LVN – Home Health
Location: Pittsfield, Massachusetts
Duration: 13 Weeks
Shift: 8 Hour Days (8:00 AM – 4:30 PM)
Guaranteed Hours: 40 Hours/Week
 

Job Summary:

Seeking an experienced Home Health LPN/LVN to provide skilled nursing care to patients in a home setting. The ideal candidate will be comfortable working independently, managing patient caseloads, and collaborating with interdisciplinary healthcare teams to ensure quality patient care and rehabilitation.

Responsibilities:

• Provide and document nursing care for patients in a home health setting
• Promote, maintain, and restore patient health through skilled nursing interventions
• Collaborate with physicians, clinicians, caregivers, and community resources to coordinate care
• Evaluate patient and family responses to treatment plans and interventions
• Maintain accurate clinical documentation and patient records
• Utilize effective communication, time management, and problem-solving skills in patient care
• Follow established clinical guidelines, protocols, and patient rights policies
• Demonstrate flexibility with assignments and scheduling needs
• Travel to patient homes and provide care in various weather conditions

Requirements:

• Minimum 1 year of acute nursing experience required
• Home Health experience required
• OASIS proficiency strongly preferred
• Infusion therapy experience preferred
• Current Massachusetts LPN license required at time of submission
• Active BLS certification required
• Valid driver’s license, auto insurance, and personal vehicle required
• Strong clinical judgment and ability to work independently
• Knowledge of home health regulations and reimbursement guidelines preferred
