Licensed Practical Nurse (LPN) – Clinic /LTC/ SNF

Job Summary

We are seeking an experienced Licensed Practical Nurse (LPN) to provide high-quality patient care in a fast-paced outpatient/clinic setting. The LPN will support physicians and RNs with patient treatments, procedures, medication administration, and clinical care coordination.

Schedule Details

• Shift Length: 8-hour shifts
• Schedule: 5 days per week
• Shift Options: Night shifts available
• 🚨 Weekend Requirement: Candidate must be available to work every other weekend, with preference given to candidates available to work every weekend (Saturday & Sunday required).

Key Responsibilities

• Assist with patient care and activities of daily living
• Administer medications and treatments as ordered
• Perform IV therapy, catheter care, and specimen collection
• Prepare patients for procedures and diagnostic testing
• Monitor patient conditions and report changes appropriately
• Maintain accurate documentation in EMR systems
• Provide patient education and emotional support

Requirements

• Active New York LPN License
• AHA BLS Certification
• IV Therapy Certification (per NY requirements)
• Strong organizational and communication skills
• Ability to work efficiently in a fast-paced clinical environment
• EPIC EMR experience preferred
