Pride Health is hiring a Physical Therapist to support our client's patients in Calico Rock, AR (Coverage Area: 50-mile radius surrounding Calico Rock). This is a 13-week assignment (Possibility of extension or conversion) and a great way to start working with a top-tier healthcare organization!

Job Name: Home Health Physical Therapist

Contract type: Local Contract

Location: Calico Rock, AR (Coverage Area: 50-mile radius surrounding Calico Rock)

Duration: 13-week Contract (Possibilities of extension)

Pay rate: $50-56/hr. On W2 (Pay Rate is based on experience)

Schedule: Mon – Fri (5 days/wk.) 8 AM to 4:30 PM (32 hrs. weekly guaranteed)

Mileage: Reimbursement provided (from first patient to last patient)

On Call/ Weekend: No

Job Responsibilities:

1. Conduct Physical Therapy evaluations for home health patients.
2. Perform reassessments to evaluate patient progress and update care plans as needed.
3. Complete PT Start of Care (SOC) visits in accordance with home health guidelines and documentation standards.
4. Provide skilled physical therapy treatment visits based on individualized patient care plans.
5. Develop and implement therapeutic interventions focused on improving mobility, strength, balance, functional independence, and safety within the home environment.
6. Accurately document patient care, assessments, progress notes, and treatment plans using Homecare Homebase (HCHB).
7. Collaborate with interdisciplinary healthcare team members to coordinate patient care and support optimal outcomes.
8. Educate patients and caregivers on exercises, mobility techniques, safety precautions, and home exercise programs.

Qualifications:

• Current Physical Therapy license in the state of practice.
• Home Health experience required.
• Experience with Homecare Homebase required.
• Current CPR certification.
• Valid driver’s license, auto insurance, and reliable transportation.
