Shift: Days – 5x8 Hour Shifts (8:00 AM – 5:00 PM)
Employment Type: Travel Contract

Job Summary:
 We are seeking an experienced Home Health Registered Nurse to provide skilled nursing care to adult and geriatric patients in the home setting. The RN will perform patient assessments, wound care, IV therapy, medication management, and ongoing education for patients with chronic and post-operative conditions.

Responsibilities:

• Perform skilled home health visits and patient assessments
• Provide cardiopulmonary and general nursing assessments
• Administer medications, SQ/IM injections, and IV therapy
• Perform wound care including wound vac and ABI compression dressing changes
• Manage PICC lines, Midlines, ports, and catheter care
• Draw labs and perform venipuncture
• Educate patients and caregivers on disease management and treatment plans
• Complete OASIS documentation and maintain accurate records
• Coordinate care with interdisciplinary healthcare teams

Requirements:

• Minimum 1 year of skilled home health RN experience
• OASIS training and experience required
• Travel experience required with more than one completed assignment
• Active BLS certification required
• Valid driver’s license and reliable transportation required
• COVID vaccine required or approved exemption
• Flu vaccine or declination required

NOTE:Private duty nursing experience alone will not be accepted

• Heart Failure (HF)
• COPD
• MI
• Pneumonia
• Stroke
• Post-operative care

Skills Required:

• IV therapy and wound care
• Foley catheter care and changes
• PICC/Midline dressing changes
• SQ port access/deaccess
• Glucometer and pro-time machine use
• Lab draws and venipuncture
• Cardiopulmonary assessment

Schedule Details:

• 6–8 home visits per day
• Monday–Friday schedules may vary:
  • 8:00 AM – 5:00 PM
  • 9:00 AM – 6:00 PM
  • Thurs–Mon or Fri–Tues rotations
• Alternating weekends required
• Self-scheduling based on office needs
