Description
Pride Health is hiring a Physical Therapist to support our client's patients in York, PA (Coverage Areas: York County & Western Adams County).
This is a 13-week assignment (Possibility of extension or conversion) and a great way to start working with a top-tier healthcare organization!
Travel Physical Therapist – Home Health
📍 York, PA 17402
💲 Local Pay Rate: $55.00 – $60.00/hour (Travel Pay weekly: $2,486.00 + M&IE + Lodging)
🕒 Weekly Guarantee: 40 Hours
Duration of the project: 3 months Contract role (May be extended depending upon the business needs)
We are seeking an experienced and compassionate Travel Physical Therapist (PT) for a home health opportunity serving York County & Western Adams County. This is an excellent opportunity for a motivated clinician who thrives in an independent setting and is passionate about delivering quality patient care in the home environment.
Position Details
- Schedule: Monday – Friday
- Shift Hours: 8:00 AM – 4:30 PM
- Productivity Expectation: 30 points/week
- Coverage Area: York County & Western Adams County
- Mileage Reimbursement: Yes – reimbursed between patient visits
- On-Call/Weekend Requirement: None
Key Responsibilities
- Perform Start of Care (SOC), evaluations, reassessments, and discharge visits
- Develop and implement individualized Plans of Care (POCs)
- Manage and coordinate an independent patient caseload
- Supervise Physical Therapist Assistants (PTAs)
- Communicate with patients and schedule daily home visits
- Deliver high-quality therapeutic interventions in accordance with physician orders and agency standards
- Complete timely and accurate documentation
Required Qualifications
- Master’s Degree in Physical Therapy or higher
- Active Physical Therapist (PT) license
- Home Health experience preferred
- Strong clinical assessment and patient care skills
- Ability to work independently and manage scheduling efficiently
Preferred Qualifications
- Experience with Homecare Homebase (HCHB)
- Knowledge of OASIS documentation
- Excellent communication and organizational skills
