Description
Position Details-:
- Mileage reimbursement as per GSA guidelines
- Common diagnosis / Types of patients/patient demographics No Peds. Most of our patients are recently discharged from the hospital and are at a high risk for rehospitalization. CHF/ COPD/ wounds/some wound vacs/ IV-PICC lines.
- How many patients do they see per day? 5.25 points per day
- Start of care 2.5 pts/ recert 1.5 pts/ resumption of care 1.75 pts/ and routine visits1 pt
- Is there a call or standby requirement? No
- Floating Requirements Home Health only
- Scrub Color/Dress Code Any work appropriate scrubs
- HomeCare HomeBase system
Required-:
- Active California RN License
- Must have 2 years of Home Health experience
- BLS eCard
- HomeCare HomeBase experience preferred
