A leading healthcare organization is actively hiring a Home Care Physical Therapist Assistant to join its growing care team serving patients throughout Gloucester, Manchester By-The-Sea, Beverly, Danvers, Peabody, Salem, Marblehead, Lynnfield, Lynn, Swampscott in Massachusetts. 

Responsibilities include:

• Provide home health physical therapy treatments and therapeutic exercises under the supervision of a licensed Physical Therapist, following the established plan of care to improve patient mobility, strength, balance, and functional independence.
• Monitor and document patient progress, response to treatment, and any changes in condition, communicating updates promptly to the supervising Physical Therapist and interdisciplinary care team.
• Educate patients and caregivers on prescribed exercise programs, safety techniques, use of assistive devices, fall prevention, and strategies to support recovery and independence in the home environment.
• Maintain accurate and timely clinical documentation, adhere to agency policies and state regulations, participate in care coordination activities, and support quality patient care across the assigned service area.

• 15-week assignments (May be extended based on business needs)
• Work Location: Wilmington, MA, USA, 01887
• Coverage Areas: Gloucester, Manchester By-The-Sea, Beverly, Danvers, Peabody, Salem, Marblehead, Lynnfield, Lynn, Swampscott.
• Will mileage be reimbursed between visits, from first patient to last patient (Y/N)? Y
• 8:00 am-4:30 pm, Monday-Friday
• On Call/Weekend Requirement: No
• Weekly Hour Guarantee: 30 hours/week
• Local Pay range: $40.00 to $43.00 an Hour

Qualifications:

• MA Physical Therapist Assistant License.
• Home Care Experience with HCHB experience.
• Type of certification required: BLS
• Minimum Education Requirement: Associate degree.
