Description
We are seeking experienced Registered Nurses for a Nurse Assessor position focused on conducting comprehensive in-home assessments for long-term care services. This is a great opportunity for RNs with strong home care and assessment experience who enjoy working independently while making a direct impact in the community.
Requirements:
• Active NY RN License required
• Minimum 2 years of recent Home Care experience within the last 10 years
• Experience with CHHA, LHCSA, MLTC, Hospice, Assisted Living, or home/community-based care
• Experience conducting assessments such as OASIS or UAS preferred
Preferred Experience:
• UAS-NY assessment experience
• Geriatric and chronic disease management experience in home settings
• Care coordination and case management experience
Responsibilities Include:
• Conducting comprehensive in-home assessments
• Reviewing medical documentation and referrals
• Developing individualized care plans
• Coordinating with interdisciplinary teams
• Educating patients on healthcare options and community resources
