Position Summary

The Registered Nurse (RN) provides overnight nursing care within a state-operated long-term care and skilled nursing facility serving adults with chronic medical, psychiatric, behavioral health, and dementia-related conditions. The RN is responsible for monitoring resident status, administering medications and treatments, responding to medical and behavioral emergencies, supervising support staff, and ensuring a safe and therapeutic environment during overnight hours.

Essential Duties and Responsibilities

• Provide direct nursing care to residents with chronic medical and behavioral health conditions during overnight hours.
• Administer prescribed medications, treatments, and therapeutic interventions accurately and safely.
• Monitor residents for changes in physical, mental, or behavioral status and communicate concerns to providers and oncoming staff.
• Perform ongoing assessments, safety rounds, and overnight monitoring of residents with dementia, psychiatric diagnoses, and complex medical needs.
• Respond promptly to medical and behavioral emergencies.
• Supervise nursing assistants and direct care staff assigned to the night shift.
• Complete documentation, nursing notes, medication records, and incident reports accurately and timely.
• Collaborate with interdisciplinary healthcare teams to support individualized resident care plans.
• Maintain compliance with infection control, safety standards, and facility policies.
• Promote a calm, secure, and therapeutic overnight environment for residents.

Qualifications

• Current Registered Nurse (RN) license in the Commonwealth of Pennsylvania.
• Graduation from an accredited nursing program.
• Strong clinical assessment, critical thinking, and communication skills.
• Experience in long-term care, behavioral health, skilled nursing, geriatrics, or dementia care preferred.
• Ability to work independently and effectively during overnight operations.
