Description

Registered Nurse (RN) – Long Term Care / Skilled Nursing Facility

 

Location: Claremont, NH

Duration:13 Weeks

Shift: Evenings

 

Key Responsibilities:

  • Monitor residents continuously for changes in physical and mental condition and report findings to nursing leadership and physicians as appropriate
  • Receive shift report and provide detailed handoff communication at change of shift
  • Accurately receive, transcribe, and implement physician orders
  • Administer medications and treatments and maintain accurate MAR/TAR documentation in electronic systems
  • Oversee medication and pharmaceutical supply management for assigned units
  • Supervise LPNs and LNAs in accordance with scope of practice and facility policy
  • Provide and support nursing care including oxygen therapy, G/J tube feedings, and wound care (including wound vacs)
  • Participate in physician rounds and resident care reviews
  • Support unit operations including therapy schedules, appointments, documentation compliance, and resident monitoring
  • Ensure infection control practices are followed and maintain a safe, clean unit environment
  • Admit, discharge, and transfer residents with appropriate documentation and notifications
  • Complete 24-hour unit reports and maintain communication with Nursing Leadership regarding resident and staff concerns
  • Communicate with residents’ families regarding updates, orders, and care coordination
  • Respond to incoming calls and determine appropriate clinical action
  • Manage assigned unit operations during shift under direction of Nursing Leadership
  • Ensure compliance with controlled substance handling, counting, and documentation requirements
  • Assist with onboarding, orientation, and education of nursing staff as needed

Requirements:

  • Minimum 3+ years of recent RN experience (within the last 6 months required)
  • Must be willing to float as needed based on experience
  • IV certification required (or willingness to obtain during onboarding)
  • Must be open to rural assignment and travel expectations
  • COVID vaccination required (no exemptions or declinations accepted)
  • Previous travel experience preferred

Required Documents:

  • 2 Supervisory References (within last 2 years)
  • Active RN License (NH or Compact)
  • Active Certifications
