Contract Length: 13 Weeks

Shift Options:

• Evening Shift: 2:00 PM - 10:30 PM
• Night Shift: 10:00 PM - 6:30 AM

Position Summary

We are seeking an experienced Licensed Practical Nurse (LPN) to join a Skilled Nursing Facility team providing quality nursing care to residents requiring short-term rehabilitation and long-term support services. The LPN will work collaboratively with interdisciplinary staff to deliver safe, compassionate, and patient-centered care while maintaining clinical excellence in a fast-paced SNF environment.

Key Responsibilities

• Provide direct nursing care to SNF residents under the direction of the RN and provider team
• Administer medications and treatments according to physician orders and facility protocols
• Monitor resident conditions and report changes promptly
• Perform routine nursing assessments and document care accurately
• Support admissions, transfers, and discharge processes
• Maintain infection prevention and patient safety standards
• Coordinate with rehabilitation, therapy, and interdisciplinary teams
• Educate residents and families regarding care plans and treatments
• Ensure compliance with facility and regulatory documentation standards

Required Qualifications

• Active New York State LPN License
• Current BLS certification
• Minimum 2 years of LPN experience (SNF or comparable setting preferred)
• Strong clinical assessment and communication skills
• Ability to work independently and prioritize resident care needs

Preferred Experience

• Skilled Nursing Facility (SNF) experience
• Long-term care or rehabilitation nursing background
• Experience managing multiple patient needs in a structured care environment
