Description

Travel Registered Nurse (RN) to provide high-quality hospice patient care within a 50-mile radius of the Baton Rouge office. This position is ideal for a skilled hospice nurse who thrives in a patient-centered environment and is experienced in coordinating interdisciplinary care.

The RN will provide direct patient care, participate in IDG meetings, coordinate medications and medical equipment, and ensure compassionate end-of-life care for hospice patients and families.
 

Schedule & Coverage

🕒 Schedule: Monday – Friday | 8:00 AM – 4:30 PM
⏰ Weekly Hour Guarantee: 36 Hours
📍 Coverage Area: 50-mile radius from Baton Rouge office
🚗 Mileage Reimbursement: Yes – reimbursed between patient visits
📞 On-Call/Weekend Requirement: Yes
 

Key Responsibilities

  • Provide direct day-to-day hospice nursing care to patients in home and facility settings.
  • Perform comprehensive patient assessments and develop individualized care plans.
  • Participate in Interdisciplinary Group (IDG) meetings and complete accurate IDG documentation.
  • Coordinate and order medications, medical supplies, and durable medical equipment (DME) as needed.
  • Educate patients and families regarding disease progression, symptom management, and hospice care services.
  • Collaborate with physicians, case managers, social workers, and hospice team members to ensure continuity of care.
  • Maintain timely and accurate clinical documentation in accordance with agency and regulatory standards.
  • Support compassionate end-of-life care while advocating for patient comfort and dignity.
  • Adhere to infection control, safety, and HIPAA guidelines.

Required Qualifications

  • Active Registered Nurse (RN) license
  • Associate Degree in Nursing (ASN) required
  • Previous Hospice nursing experience required
  • Ability to work independently in field-based settings
  • Strong communication, organizational, and clinical assessment skill
  • Valid driver’s license and reliable transportation
